Chart Smart

If your patient has a heat-related health alteration, make sure you document the following information:

• prescribed and administered medications
• cooling procedures and their effect
• intake and output
• level of consciousness
• cardiac output
• vital signs
• heart sounds
• lung sounds
• central venous pressure and pulmonary artery wedge pressure if central line in place
• oxygen administration
• patient teaching.
