Chart Smart

If your patient has hyperphosphatemia, make sure you document the following information:

• all assessment findings
• intake and output
• IV therapy and medications given
• muscle spasms, cramps, pain, and muscle strength
• paresthesia in the fingertips and around the mouth
• visual disturbances
• safety measures to protect patient
• notification of the practitioner
• your interventions, including patient teaching, and the patient's response.
