Chart Smart

If your patient has respiratory alkalosis, make sure you chart the following information:

• vital signs
• intake and output
• IV therapy
• interventions, including measures taken to alleviate anxiety
• patient's response to interventions
• ABG and serum electrolyte levels
• safety measures
• notification of the practitioner
• patient teaching.
