- Sexual activity in patients with preexisting cardiovascular disease may pose a risk of cardiac events. Sildenafil should not be used if sexual activity is inadvisable because of their underlying cardiovascular status
- Vardenafil has vasodilatory properties. Patients with left ventricular outflow obstruction, e.g, aortic stenosis and idiopathic hypertrophic subaortic stenosis, could be adversely affected by vasodilatory effects
- Use with caution if coadministered with alpha-blockers because of risk of additive reduction in BP. Significant symptomatic hypotension may occur
- Prolonged erection > 4 hours and priapism (painful erections greater than 6 hours in duration) have been reported
- Safety and efficacy in the following conditions has not been established
- Severe hepatic impairment
- End stage renal disease requring dialysis
- Hypotension (SBP< 90 mmHg)
- Uncontrolled hypertension (BP > 170/110) (Safety or efficacy not established)
- MI within 6 months (Safety or efficacy not established)
- Stroke within 6 months (Safety or efficacy not established)
- Life-threatening arrhythmia in last 6 months (Safety or efficacy not established)

- Retinitis pigmentosa
- Retinal disorders, degenerative

- Strong inhibitors of the CYP3A4 enzyme system

- Non-arteritic anterior ischemic optic neuropathy
- Bleeding disorder
- PUD, active
- Elderly patients
