Critical Element Criteria

Students must meet criteria of Critical Elements to safely perform the skill based on evidence-based practice. Score can be pass/fail, letter grade, or point assignment. Only acceptable score for Critical Elements is 100%

Critical Elements

  • Communicates clearly with caring behavior.
  • Perform hand hygiene before and after procedure.
  • Use of personal protective equipment when needed.

Procedure

Prior to Treatment: (Critical Thinking)

  • Gather equipment: physical assessment equipment appropriate to patient situation.

Patient Interaction: (Safety)

  • Explain procedure.
  • Use two patient identifiers.
  • Wash hands.

Steps of the Procedure: (Patient Centered Care)

  1. Introduce self to patient and family.
  2. Assist patient into hospital gown.
  3. Measure weight, and transfer patient to bed.
  4. Evaluate vital signs and admission assessment including health history and physical assessment.
  5. Explain equipment, including call light and location of personal care items.
  6. Explain hospital routines, including use of side rails, meal times, etc. Answer patient and family questions.
  7. Advise patient of privacy rights.
  8. Complete inventory of belongings, encourage family to take home valuable items. If that is not possible, arrange for valuables to be placed in hospital safe.
  9. Ensure that admission orders have been complete.

After Procedure (Patient Centered Care)

  • Straighten room making patient comfortable, replace call light.
  • Wash hands before leaving room.

Document (Informatics, Communication)

  • Complete admission paperwork.
