Adult Dosing
Moderate musculoskeletal discomfort
- Start 1000 mg IM/IV; may repeat dose q8hrs up to a maximum of 3000 mg/day for 3 consecutive days
Adjunct therapy for tetanus
- 1000-2000 mg IV; an additional 1000-2000 mg may be added to infusion up to 3000 mg/day
- Repeat q6hrs until oral form/NG form may be administered
Notes:- Switch to PO/NG when feasible
- Do not dilute one vial given as a single dose to more than 250 mL for IV infusion
- Do not inject more than 5 mL into each gluteal region when IM route is indicated
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established except in tetanus
Adjunct therapy for tetanus
- 500 mg/m2/day IV q6 hrs PRN
- Alt: 15 mg/kg IV q6 hrs PRN
- Max: 1800 mg/m2/day for <3 consecutive days

- For intravenous and intramuscular use only; do not use for subcutaneous administration
- Methocarbamol may possess a general CNS depressant effect; caution patients against the use of alcohol and other CNS depressants, as this may aggravate the symptoms
- Methocarbamol injectable can cause fetal harm. Unless the potential benefits outweigh the possible hazards, do not use in women who are or may become pregnant and particularly during early pregnancy
- Methocarbamol may impair mental and/or physical abilities required for performance of hazardous tasks; caution patients against operating machinery, including automobiles, until they are reasonably certain that the therapy does not adversely affect their ability to engage in such activities
- Use cautiously in patients with known or possible latex sensitivity, as the vial stopper contains dry natural rubber that can cause hypersensitivity reactions
- Administer methocarbamol slowly at a rate not exceeding 3 mL/min (10 mL in 3 minutes). The patient should remain in a recumbent position during and for at least 10 to 15 min following the injection to reduce the likelihood of side reactions like extravasation
- Do not exceed the dose of 30 mL/day for more than three consecutive days except in the treatment of tetanus

- Advise patients not to perform hazardous tasks which require mental alertness such as operating machinery or driving a motor vehicle
- Caution patients regarding the combined effects with alcohol and other CNS depressants; advise not to use with alcohol and other CNS depressants
