Signs and Symptoms

Si + Sx:Seizures (15%), Vtach (20%) in all >60 µgm/mL, low K+ (100%), elevated glucose (98%), low PO4 (80%), low Mg2+ (75%)

Lab and Xray

Lab:Toxicity with levels >35 µgm/mL

Treatment

Rx:

Charcoal; hemoperfusion over charcoal if level >100 µgm/mL in acute, >40-60 µgm/mL in chronic toxicity, one of few indications for hemoperfusion (Med Let 1986;28:80); dialysis if hemoperfusion not available

of tachycardia, hypotension, and ventricular arrhythmias: esmolol; or low-dose propranolol, eg, 0.5 mg iv in adults (0.01 mg/kg to max of 1 mg/dose in children), repeat in 5-10 min if BP and p not decr; complete beta.gif blockade occurs at 0.2 mg/kg
