- The same adverse reactions found with systemic administration of beta-adrenergic blocking agents may occur with topical administration.
- Severe respiratory reactions and cardiac reactions, including death due to bronchospasm in patients with asthma, and death due to cardiac failure, have occurred on ophthalmic administration
- Sympathetic stimulation is essential for support of the circulation in individuals with diminished myocardial contractility, and its inhibition by beta-adrenergic receptor blockade precipitates more severe failure
- In patients without a history of cardiac failure having continued depression of the myocardium with beta adrenergic receptor inhibitors over a period of time leads to cardiac failure. Discontinue this drug on first sign or symptom of cardiac failure
- Avoid administration of this drug in patients with chronic obstructive pulmonary disease of mild or moderate severity, bronchospastic disease, history of bronchospastic disease and in which timolol is contraindicated
- Impairs the ability of the heart to respond to beta adrenergically mediated reflex stimuli; may augment the risk of general anesthesia in surgical procedures. Protracted, severe hypotension has occurred in some cases during anesthesia. Difficulties in restarting and maintaining the heartbeat have occurred. Consider gradual withdrawal of beta adrenergic receptor inhibitors in patients undergoing elective surgery
- Therapy with this drug may mask the signs and symptoms of acute hypoglycemia, hyperthyroidism
- Carefully manage patients suspected of developing thyrotoxicosis
- Consider alternative therapy if signs or symptoms suggest reduced cerebral blood flow following initiation of therapy with timolol
- Bacterial keratitis have occurred with use of multiple dose containers of topical ophthalmic products when these containers are inadvertently contaminated by patients who had a concurrent corneal disease or a disruption of the ocular epithelial surface
- Choroidal detachment after filtration procedures has occurred with the administration of aqueous suppressant (e.g., timolol)
- Timolol exhibits less or no effect on the pupil and avoid using it alone in the treatment of angle closure glaucoma
- Patients having history of atopy or a history of severe anaphylactic reactions to a variety of allergens and treated with this drug are more reactive to repeated accidental, diagnostic, or therapeutic challenge with such allergens; such patients are unresponsive to the usual doses of epinephrine used to treat anaphylactic reactions
- Muscle weakness consistent with certain myasthenic symptoms have occurred

Breastfeeding: Timolol is excreted variably in breast milk; prefer other drugs especially while nursing a newborn or preterm infant. Ophthalmic use may pose little risk to the breastfed infant. To substantially diminish the amount of drug reaching the breastmilk after using eye drops, place pressure over the tear duct by the corner of the eye for 1 minute or more and remove the excess solution with an absorbent tissue.
