Pathophys:Conjunctival infection from maternal genital infection

Epidemiology

Onset in 1st 28 d of life

Signs and Symptoms

Si:Exudative conjuctivitis in neonate

Complications

Blindness, common w GC, rare w chlamydia

r/o Herpes simplexif ulcerations

Lab and Xray

Lab:

Bact:Gram stain will show GC, not chlamydia

Treatment

Rx:

Prevent w prepartum culture screens, and/or prophylaxis at birth w 2.5% povidone iodone (5% soln diluted 50:50) (Nejm 1995;332:562), which is less toxic and more effective than 1% silver nitrate, 1% tetracycline ointment, or 14% erythromycin ointment

of chlamydial disease: systemic erythromycin 50 mg/kg divided qid × 14 d 80% cure (MMWR Recomm Rep 2002;51[RR-6]:1), along w topicals
