Adult Dosing
Treatment of iron deficiency anemia
- 60-100 mg elemental iron PO bid
- ALT: 2-3 mg/kg/day PO divided bid-qid
As iron supplement
Note:
- Ferrous gluconate contains 12% elemental iron
- Dosages are expressed as mg of elemental iron
Pediatric Dosing
Treatment of iron deficiency anemia
Children and Infants
- Severe iron deficiency anemia: 4-6 mg/kg/day PO divided tid
- Mild to moderate iron deficiency anemia: 3 mg/kg/day PO divided qd-bid
Neonates, premature
- 2-4 mg/kg/day divided qd-bid [Max: 15 mg/day]
Prophylaxis
- 1-2 mg/kg/day PO divided qd-bid [Max: 15 mg/day]

- Long term administration of iron can cause hemosiderosis leading to excessive iron absorption, excess tissue iron stores, and potential tissue injury and as there is no excretory mechanism, repeated phlebotomy or long-term administration of deferoxamine is necessary to prevent or reverse tissue damage
- Iron overload is associated with an increased susceptibility to certain infections (caused by Vibrio vulnificus, Yersinia enterocolitica, or Y. pseudotuberculosis)and can adversely affect the prognosis in patients infected with human immunodeficiency virus (HIV)
- As premature infants have low serum vitamin E concentrations, administration of iron preparations can cause increased red cell hemolysis and hemolytic anemia, IM administration of the vitamin may be advisable
- Except in patients with continued bleeding, menorrhagia, or repeated pregnancies, long term (more than 6 months) administration of iron should be avoided
- Accidental overdose of iron-containing products is a leading cause of fatal poisoning in children less than 6 yrs of age.
