Pathophys and Cause

Cause:Haemophilus ducreyi(Ann IM 1981;95:315)

Epidemiology

Venereally spread, ie, an STD, associated with syphilis in 15%

Signs and Symptoms

Sx:Painful ulcer after 2- to 15-d incubation

Si:Adenopathy; tender, "dirty," necrotic ulcer(s); tender bubos (40%)

Course

Self-limited

Lab and Xray

Lab:Bact:Culture positive in 80%

Treatment

Rx:

for patient and contacts: ceftriaxone 250 mg im once; or azithromycin 1 gm po once or, ciprofloxacin 500 mg po bid × 3 d; or erythromycin 500 mg qid × 7 d

in AIDS, azithromycin, cipro, or erythro as above
