Adult Dosing
Hypertension
- Initial: 1 mg PO daily a.m. or p.m.
- Titrate: Depending on individual standing blood pressure response 2-6 and 24 hr post dose,increase to 2 mg and then increase q2 weeks to next highest dose as needed
- MAX: 16 mg/day
- Note: Doses > 4 mg increase likelihood of excessive postural effects
Benign prostatic hyperplasia
- Immediate release
- Initial: 1 mg PO Daily a.m. or p.m.
- Titrate: Depending on individual urodynamics and BPH symptomatology, increase q2 weeks to next highest dose as needed
- MAX: 8 mg/day
- Extended release
- Start 4 mg PO Daily
- May increase to 8 mg/day after 3-4 wks
- Max: 8 mg/day
Pediatric Dosing
- Safety and effectiveness in pediatric population has not been established

- Doxazosin may cause marked hypotension, especially in upright position, with syncope with sudden loss of consciousness. This usually occurs with first dose of the drug but may also occur with rapid dosage increases or if therapy is interrupted for more than a few days. Start therapy at a lower dosage and increase gradually.
- Make patient aware of possible first-dose syncope
- If syncope occurs, position patient in a recumbent position and treat supportively as necessary
- Caution patients being titrated with doxazosin to avoid situations where injury could result should syncope occur, during both the day and night
- Careful monitoring of postural blood pressure during initial titration or subsequent upward adjustment in dosage is recommended
- Rule out carcinoma of the prostate before starting therapy for BPH
- Intraoperative floppy iris syndrome has been observed during cataract surgery in some patients
- Rarely, doxazosin may cause priapism
- Assess changes in urinary symptoms, such as dribbling, hesitancy, frequency, nocturia, volume, weak stream
- Avoid driving or performing hazardous tasks for the first 24 hours after taking doxazosin or when the dose is increased
Cautions: Use cautiously in
- Hepatic impairment
- Hypotension
- Cataract surgery
- Chronic constipation (for XL form)
- Severe GI stricture (for XL form)
Pregnancy Category:C
