Adult Dosing
Hypertension
- Start 80-160 mg PO qd if on monotherapy (in patients with no volume depletion); use lower dose if on diuretic
- May increase to 160-320 mg/day
- Range: 80-320 mg PO daily
- Max: 320 mg/day
Heart failure (NYHA Class II-IV)
- Start 40 mg PO bid; increase to 160 mg bid as tolerated
- Max: 320 mg/day PO in divided doses
Note: Decrease concomitant diuretic dose
Left ventricular dysfunction, post MI
- Start 20 mg PO bid
- Increase within 7 days to 40 mg PO bid followed by subsequent titration to target dose of 160 mg bid
- Max: 320 mg/day PO divided bid
Note: Initiate therapy 12 hrs post MI
Pediatric Dosing
Hypertension
Child 6-16 yrs
- Start 1.3 mg/kg PO to 40 mg PO daily
- May increase to 2.7 mg/kg PO daily
- Max: 160 mg/day

- Drugs that act on the renin angiotensin system can cause fetal and neonatal morbidity and mortality when used in pregnancy. Discontinue drug as soon as pregnancy is detected
- Hypotension may occur in in heart failure or post-MI patients. Use with caution when initiating therapy.
- Symptomatic hypotension may occur after initiation of valsartan therapy in patients with intravascular volume depletion (eg, those treated with diuretics). Correct these conditions prior to starting therapy with valsartan or start treatment under medical supervision
- Patients with renal artery stenosis may experience acute renal failure. In patients with severe heart failure whose renal function may depend on the activity of the renin-angiotensin-aldosterone system, treatment may be associated with oliguria and/or progressive azotemia and (rarely) with acute renal failure and/or death. Use with caution in severe CHF
