Adult Dosing
Atopic dermatitis
- Apply 0.1% cream to affected areas qd x 2 wks
- Max: 60 g/wk
Corticosteroid-responsive Dermatophyses
- Apply 0.05% cream/ointment/gel to the affected area/s as a thin film bid-qid, depending on the severity of the condition
- Alt: Apply 0.1% cream to affected areas bid x 2 wks; Max: 60 g/wk
Plaque psoriasis
- Apply 0.1% cream to affected areas qd or bid x 2 wks
- Max: 60 g/wk
Pediatric Dosing
- Note: Safety and efficacy in pediatric patients < 12 yrs have not been established

- Do not discontinue abruptly, switch to a lesser potent drug; patients receiving large doses applied to a larger surface area or under occlusive dressings should be evaluated periodically for evidence of HPA axis suppression by using urinary free cortisol and ACTH stimulation tests. If HPA axis supression noted, withraw and substitute with lower potency drug
- Inhibit HPA axis; systemic absorption of fluocinonide may produce reversible hypothalamic pituitary adrenal (HPA) axis suppression, Cushing's syndrome, hyperglycemia and unmask latent diabetes
- Systemic absorption may require evaluation for HPA axis suppression
- Potent corticosteroids, use on large areas, prolonged use or occlusive use may increase systemic absorption
- Local adverse reactions with topical steroids may include atrophy, striae, irritation, acneiform eruptions, hypopigmentation and allergic contact dermatitis and may be more likely to occur with occlusive use or more potent corticosteroids
- Children may be more susceptible to systemic toxicity due to their larger skin surface to body mass ratios when treated with topical corticosteroid
- Discontinue or reduce dose or potency if HPA axis suppression, Cushing's syndrome, hyperglycemia, glucosuria, or irritation occurs
- If concomitant skin infections are present/develop, an appropriate antifungal/antibacterial agent should be used; discontinue if infection persists or worsens
Cautions: Use cautiously in
- Long term use (may cause atrophy of the skin and subcutaneous tissues)
- Occlusive dressings (adverse effects more frequent)
- Diabetes mellitus
- Cataracts, glaucoma, or tuberculosis (concommitant use of high-potency drugs may worsen condition)
- Pre-existing skin atrophy
Supplemental Patient Information
- Avoid contact with eyes and prolonged facial application; do not bandage the treated area or use more than instructed
Pregnancy Category:C
Breastfeeding: Safety unknown; literature on the use of topical fluocinonide during breastfeeding is unavailable. Due to poor absorption on topical application considered as a low risk to the nursing infant. Ensure that the infant's skin does not come into direct contact with the treated areas of skin. Apply only water-miscible cream products to the breast because ointments may expose the infant to high levels of mineral paraffins via licking.
