Adult Dosing

Hypertension

• Initial dose: 1 mg PO bid-tid
• Maintenance dose: 6-15 mg/day PO in divided doses
• MAX: 20 mg/day PO in divided doses
• Note: If a diuretic or other antihypertensive agent is added, reduce dose to 1-2 mg PO tid and retitrate

Benign prostatic hyperplasia (not FDA approved)

• 0.5-1 mg PO bid
• MAX: 2 mg PO bid

Prophylaxis of autonomic dysreflexia (Non-FDA Approved)

• 0.5–1 mg PO q8 hrs

• Prazosin is indicated for the management of hypertension (either alone or in combination with other antihypertensive drugs such as diuretics or beta-adrenergic blocking agents)

• Known sensitivity to quinazolines, prazosin, or any of the ingredients

• Prazosin may cause syncope with sudden loss of consciousness. Syncopal usually occurs within 30 to 90 minutes of the initial dose of the drug. May also occur with rapid dosage increases or the introduction of another antihypertensive drug into the regimen. start therapy at a lower dosage and increase gradually
• Make patient aware of possible first-dose syncope
• Careful monitoring of blood pressure during initial titration or subsequent upward adjustment in dosage of prazosin is recommended
• First dose should be started in the night
• Intraoperative floppy iris syndrome has been observed during cataract surgery in some patients
• Avoid driving or performing hazardous tasks for the first 24 hours after taking prazosin or when the dose is increased

• CNS: Dizziness, headache, drowsiness, weakness, mental depression, syncope, vertigo
• CV: First-dose orthostatic hypotension , palpitations, edema, dyspnea, syncope
• EENT: Blurred vision, reddened sclera, epistaxis, dry mouth, nasal congestion
• DERM: Rash
• GI: Vomiting, diarrhea, dry mouth, nausea, abdominal cramps
• GU: Urinary frequency, erectile dysfunction, priapism

• Acts by blocking postsynaptic alpha1-adrenergic receptors (peripherally-acting alpha blocker)
• Metabolized by liver to active metabolites
• Excreted in bile/feces primarily, urine: 6-10% unchanged
• Half-life: 2-3 hrs
