Corticosteroid-responsive dermatoses
Cream
- Apply a thin layer to the affected skin areas bid; rub in gently. Max: 4 wks
Ointment
- Apply a thin layer to the affected skin areas bid; rub in gently. Max: 4 wks
Atopic dermatitis
Lotion
- Apply a thin layer to the affected skin areas qd for up to 4 wks
Note: Re-assess the patient if there is no improvement after 2 wks
Pediatric Dosing

- Systemic absorption can produce reversible hypothalamic-pituitary-adrenal (HPA) axis suppression; potential for glucocorticosteroid insufficiency after withdrawal from treatment. Periodically evaluate for HPA axis suppression while applying topical fluticasone to larger surface area and areas under occlusion. Withdraw or substitute with a less potent corticosteroid if HPA axis suppression is noted
- Pediatric patients are at a greater risk of adrenal insufficiency during or after withdrawal of treatment. Due to a higher ratio of skin surface area to body mass, they are also at a greater risk of HPA axis suppression and Cushing's syndrome than adults
- Do not use ointment in pediatric patients as safety is not established
- Use lowest effective dose and potency especially in children
- Restrict use of lotion in patients with hypersensitivity to formaldehyde as it may cause allergic sensitization or irritation of the skin
- Institute appropriate antifungal or antibacterial therapy in case of pre-existing skin infection
- Allergic contact dermatitis may occur; discontinue use if skin irritation develops
- Do not use in pre-existing skin atrophy and infection at treatment site
- Do not use for treatment of rosacea and perioral dermatitis
- Avoid occlusive dressing

- Advise parents not to use this medication in the treatment of diaper dermatitis
- Do not use near eyes or pre-existing skin atrophy

Pregnancy Category:C
Breastfeeding: Topical fluticasone has not been studied during breastfeeding. Short-term application of topical corticosteroids is not expected to pose a risk to the breastfed infant by passage into breastmilk. Use of the least potent drug on the smallest area of skin of mother is recommended. Ensure that the infant's skin does not come into direct contact with the areas treated with this drug. Use of lower potency corticosteroids, water-miscible cream products on the nipple or areola is recommended. Any topical corticosteroid should be wiped off thoroughly prior to nursing if it is applied to the nipple or areola. According to manufacturer's data, it is unknown whether topical administration of the drug could result in sufficient systemic absorption to produce detectable quantities in human milk.
