Pathophys and Cause

Cause: Chlamydia trachomatis, immuno type L

Epidemiology

Venereally spread. Highest incidence in gay males. Male/female = 20:1

Signs and Symptoms

Sx:Painless papule (30%); bubo (70%); long incubation period

Si:Adenopathy, local and distant; fever; erythema nodosum; ulcerative proctitis, in gays (Nejm 1981;305:195)

Complications

Rectal stricture; cancer develops in 2%

Lab and Xray

Lab:

Rv of dx tests (Nejm 1983;308:1563)

Bact:Culture possible and quite easy

Serol: Comp-fix antibody pos (>1/16) in 60%, r/o psittacosis

Treatment

Rx:

Tetracycline × 21 d, often must repeat × 1-2; or erythromycin 500 mg qid × 21 d
