Kidney injury-trauma grading
Kidney injuries are graded I-V with increasing severity having higher grading.
Grade I
- Renal contusion with microscopic or gross hematuria, urologic studies normal
- Renal subcapsular hematoma that is nonexpanding, with no parenchymal laceration
Grade II
- Nonexpanding perirenal hematoma that is confined to renal retroperitoneum
- Renal laceration that is <1.0 cm parenchymal depth of renal cortex without urinary extravagation
Grade III
- Renal laceration that is >1.0 cm parenchymal depth of renal cortex without collecting system rupture or urinary extravasation
Grade IV
- Renal parenchymal laceration that extends through the renal cortex, medulla, and collecting system
- Renal vascular injury to the main renal artery or vein injury with contained hemorrhage
Grade V
- Completely shattered kidney
- Renal vascular injury with avulsion of renal hilum which devascularizes the kidney
Note to advance one grade for bilateral injuries or multiple injuries to the same organ up to grade III.
