Pediatric Dosing

- Increased risk of hyperkalemia, especially when used concomitantly with an angiotensin-converting enzyme inhibitor, an angiotensin II receptor antagonist, cyclosporine or tacrolimus. Monitor for warning signs/symptoms of hyperkalemia including paresthesias, muscular weakness, fatigue, flaccid paralysis of the extremities, bradycardia, shock, and ECG abnormalities. Monitor serum potassium at baseline, dose changes, when illness alters renal function, and periodically [US Black Box Warning]; withhold dose if Serum potassium levels > 5.5 mEq/L,
- Use with extreme caution in diabetes due to risk of hyperkalemia
- Electrolyte imbalances, including hyperkalemia, hyponatremia, hypochloremia, and increases in BUN may occur
- In severe liver disease, hepatic encephalopathy (eg, tremors, confusion, coma, jaundice) may occur
- If ECG changes occur (abnormal S or P waves, peaked T waves), withhold dose
- Monitor BUN/Cr at baseline, then periodically, or more frequently if renal impairment, diabetes, elderly, or seriously ill
- Avoid co-administration of Lithium because of high risk of lithium toxicity

- Renal Impairment (measure BUN/CrCl at baseline and at regular intervals)
- Hepatic impairment
- Diabetes mellitus (monitor serum electrolytes and renal function)
- Cardiopulmonary disease
- Potassium sparing diuretic use (frequently monitor serum potassium)
- Debilitated patients
- Geriatric population
