Rectum injury-trauma grading

Rectal injuries are graded I-V with increasing severity having higher grading.

Grade I

• Hematoma or contusion without devascularization
• Rectal laceration that is partial thickness

Grade II

• Rectal laceration <50% of circumference

Grade III

• Rectal laceration with 50% circumference

Grade IV

• Rectal laceration that is full thickness with extension into the perineum

Grade V

• Rectal vascular injury with devascularized segment

Note to advance one grade for multiple injuries up to grade III.
