Left Trapezius (At the midpoint of the upper border)
Right Trapezius (At the midpoint of the upper border)
Left Supraspinatus (At the origin, above the scapular spine near the medial border)

Left Second rib (At the second costrochondral junction, just lateral to the junction on the upper surface)

Fibromyalgia Criteria (Acr 1990 Criteria)

The AmericanCollege of Rheumatology issued criteria for the diagnosis of fibromyalgia in 1990; it is these criteria that are cited below.

Fibromyalgia is a chronic clinically diagnosed condition with patients that commonly have:

  • Pain "all over"
  • Poor energy
  • Sleep disturbances
  • Multiple areas on the body that are painful to even mild pressure
  • More common in females
  • Headaches
  • Irritable bowel
  • Depression
  • Paresthesias
  • Mood changes
  • Anxiety
  • Negative medical workup for other explanations of these symptoms
  • Prevalence is 2-5% and onset

 

BOTH Criteria #1 and at least 11 of 18 items in Criteria #2 must be present to make the diagnosis.

Criteria #1

History of widespread pain for at least 3 months (All of the following must be present):

  • Pain in the left side of the body
  • Pain in the right side of the body
  • Pain above the waist
  • Pain below the waist
  • Axial skeletal pain (cervical spine, anterior chest, thoracic spine, or low back)

Criteria #2

Pain on digital palpation (should use ~4 kilograms of force) in at least 11 of these 18 sites (9 points that are bilateral). To be a positive finding; the digital palpation must be "Painful" not just "Tender."

  • Occiput (At the suboccipital muscle insertion)
  • Low cervical (At the anterior aspect of the intertransverse spaces at C5-C7)
  • Trapezius (At the midpoint of the upper border)
  • Supraspinatus (At the origin, above the scapular spine near the medial border)
  • Second rib (At the second costrochondral junction, just lateral to the junction on the upper surface)
  • Lateral epicondyle (2 cm distal to the epicondyle)
  • Gluteal (In the upper outer quadrant of the buttock in the anterior fold of the muscle)
  • Greater trochanter (Posterior to the trochanteric prominence)
  • Knee (At the medial fat pad proximal to the joint line)
