Introduction

Desaturated phosphatidylcholine (DSPC) is the major component (50%) of fetal pulmonary surfactant. The concentration in amniotic fluid can be measured by separating DSPC from unsaturated lecithin. Phosphatidylcholine is the second most surface-active component of surfactant.

This test is a direct measure of primary phospholipid in surfactant and is used in the assessment of FLM.

Procedure

Obtain and examine amniotic fluid for the DSPC.

Clinical Implications

  1. Normal levels are consistent with FLM and indicate a negligible risk for RDS.

  2. Low levels are associated with immaturity and a high risk for RDS.

Interventions

Pretest Patient Care

  1. Explain test purpose and the amniotic fluid sampling procedure.

  2. Follow guidelines in Chapter 1 for safe, effective, informed pretest care.

Posttest Patient Care

  1. Review test results; report and record findings. Modify the nursing care plan as needed. Monitor the patient and counsel appropriately.

  2. Follow guidelines in Chapter 1 for safe, effective, informed posttest care.

Interfering Factors

Results may be altered by changes in AFV (oligohydramnios or polyhydramnios).
