Acute hemolytic transfusion reaction (AHTR) is triggered by an antigen–antibody reaction and activates the complement and coagulation systems. These are most always due to ABO incompatibility because of misidentification resulting in the patient receiving incompatible blood. Symptoms include fever, chills, backache, vague uneasiness, and red urine. AHTR is life-threatening.
