Drug Therapy of Haemodynamically Stable Monomorphic Ventricular Tachycardia

Efficacy in converting VT uncertain

Major role is in suppression of recurrent episodes

Needs to be given via central vein to avoid thrombophlebitis

Loading: 300 mg, diluted in 5% glucose to a volume of 20–50 mL, infused over 20 min via a central vein

Maintenance: 900–1200 mg over 24h

Supplementary doses of 150 mg can be given as necessary every 10 min for recurrent or resistant

VT to a maximum total daily dose of 2.2g

Lidocaine

Converts 30%

Appropriate when VT is due to myocardial ischaemia or infarction

May cause hypotension and neurological side effects

Loading: 1.5 mg/kg over 2 min

Maintenance: 1–4 mg/min
