* A dose of 150 mg OD for thromboprophylaxis or 110 mg BD for AF/recurrent VTE should be considered in patients >75 years or patients with eGFR <50 mL/min.
y The reduced doses of apixaban and dabigatran in atrial fibrillation are recommded for patients with two of the following: age >60 years, weight<60kg, serum creatinine >133micromol/L.
