Some Atypical Presentations of Stroke

Brainstem | Isolated cranial nerve disorders, vertigo, crossed motor and sensory signs, ataxia |
Frontal lobe | Change in behaviour/personality without focal neurological signs |
Localized language area | Mobile aphasic patient often initially diagnosed as delirium/dementia |
Bilateral thalamic infarction | Confusion |
Pons | Locked in syndrome |
Cortical lesions | Focal seizures |
Basal ganglia | Abnormal movements, for example asterixis, hemifacial spasms |
Visual cortex | Isolated visual symptoms, for example Anton's syndrome, Balint's syndrome |
Frontal or posterolateral parietal lobe | Alien hand syndrome |
