Contraindications to Thrombolysis for Cerebral Infarction

  • Evidence of intracerebral haemorrhage or previous intracerebral haemorrhage
  • Time of onset 6h or more before treatment or evidence on the scan of established recent infarction
  • Uncontrolled hypertension (above 185/110 mmHg)
  • Witnessed seizure at stroke onset
  • Taking warfarin with INR >1.7, or a direct-acting oral anticoagulant
  • Bleeding diathesis
  • Severe previous neurological disability
  • Arterial puncture or bleeding at non-compressible site within last 7 days
  • Within 14 days of major surgery
  • Rapidly improving symptoms
