Transmucosal lozenge: 200, 400, 600, 800, 1200, 1600 µg
Inj: 50 µg/ml
Patch: release 25, 50, 75, 100 µg/h
Conscious sedation: Child 2-12y.o.: 2-3 µg/kg IV over 3 min Q 30 min prn. Child > 12y.o.-adult: 50-100 µg over 3 min Q 30 min prn.
Chronic pain: Adult: 1 patch Q 72h. Start patients who are not tolerant of opiates with 25 µg/h patch.
Breakthrough cancer pain: 200 µg transmucosal unit over 15 min. May repeat in 30 min, max 4 U/d.
