Norton Scale for Assessing Risk of Pressure Ulcers (Calculator)

The Nortion scale uses following five criteria to assess patient's risk for pressure ulcers:

  1. Physical Condition
  2. Mental Condition
  3. Activity
  4. Mobility
  5. Incontinent

Scores of 14 or less indicate liability to ulcers; scores of <12 indicate very high risk.
