Adult Dosing
Hyperphosphatemia
Renagel/Renvela Tabs
- Starting dose
- Start 800 mg PO tid if serum phosphorus >5.5 and <7.5 mg/dL
- Start 1200-1600 mg PO tid if serum phosphorus
7.5 and <9 mg/dL - Start 1600 mg PO tid if serum phosphorus >9 mg/dL
- Max: 13 g/day
- Maintenance doses
- Increase/decrease by 800 mg tablet tid q2 wks interval to keep serum phosphorus levels
5.5 mg/dL
Note: Give with meals. Give other drugs 1 hr before or 3 hrs after
Renvela Powder
- Starting dose
- 0.8 gm PO tid if serum phosphorus >5.5 and <7.5 mg/dL
- 1.6 gm PO tid if serum phosphorus 7.5
- Maintenance dose
- 0.8 mg tid q2 wks interval to keep serum phosphorus levels 5.5 mg/dL
Note: 0.8/2.4 gm of Sevelamer Carbonate should be mixed with 30/60 mL water, drink the entire preparation within 30 minutes
Pediatric Dosing
- Safety and effectiveness in pediatric patient have not been established

- Safety and effectiveness of sevelamer is not established in patients with GI motility disorders, dysphagia, or major GI tract surgery, use with caution
- Monitor folic acid, 25-hydroxycalciferol and vitamins D, E, K levels periodically
- Monitor serum phosphorus, bicarbonate and chloride levels
- Patients on peritoneal dialysis should be closely monitored
Cautions: Use cautiously in
- Dysphagia
- GI motility disorder
- Severe constipation
- Major GI tract surgery
Pregnancy Category:C
