Adult Dosing

Candidemia

• Loading dose: 200 mg IV x 1 on day 1
• Maintenance dose: 100 mg IV q24 hrs thereafter for at least 14 days after the last positive culture

Esophageal Candidiasis

• Loading dose: 100 mg IV x 1 on day 1
• Maintenance dose: 50 mg IV q24 hrs for a minimum of 14 days and at least 7 days after the last positive culture

Other candida infections

• Loading dose: 200 mg IV x 1 on day 1
• Maintenance dose: 100 mg IV q24 hrs thereafter for at least 14 days after the last positive culture

Pediatric Dosing

• Safety and effectiveness in pediatric patients has not been established

Renal Dose Adjustment

• Renal impairment: No dose adjustments
• Hemodialysis: No supplement

Hepatic Dose Adjustment

• Hepatic impairment: No dose adjustments

• Isolated cases of hepatic dysfunction, hepatitis, or worsening hepatic failure have been reported in patients receiving anidulafungin
• Monitor patients developing abnormal LFTs during therapy for worsening hepatic function and evaluate need for continued therapy
• Anaphylactic reactions, including shock, may occur with anidulafungin. If such reactions occur, discontinue the treatment and take appropriate measures. Infusion-related adverse reactions, including rash, urticaria, flushing, pruritus, bronchospasm, dyspnea, and hypotension have been reported with therapy. To reduce these events, do not exceed a rate of anidulafungin infusion of 1.1 mg/minute

• Antifungal; inhibits the synthesis of fungal cell wall. Noncompetitive inhibitor of 1,3-beta-D-glucan synthase leading to decreased 1,3-beta-D-glucan, a 30% to 60% component of Candida cell wall (absent in mammalian cells), resulting in osmotic instability and cellular lysis
• Metabolized by chemical degradation at physiologic temperature and pH
• Excreted in feces 30% (< 10% unchanged) and urine < 1%
• Half-Life: 40-50 hrs
