Hypertension
Monotherapy (used alone or in combination)
- Initial: 16 mg PO qd; use lower dose if volume depletion or if using a diuretic
- Usual: 8-32 mg PO/day divided qd-bid
- Max: 32 mg/day
CHF
- Start 4 mg PO qd
- Double dose q2 wks
- Max: 32 mg/day
Pediatric Dosing
Hypertension
Child (1 to <6 years)
- Starting dose: 0.20 mg/kg oral suspension PO qd
- Range: 0.05 - 0.4 mg/kg/day oral suspension once daily or consider divided dose
Child (6 to <17 years)
- Starting dose
- <50 kg: 4 8 mg tablet PO qd
- >50 kg: 8 16 mg tablet PO qd
- Dose Range
- <50 kg: 4 16 mg tablet PO qd or consider divided dose
- >50 kg: 4 32 mg tablet PO qd or consider divided dose

- Symptomatic hypotension may occur in patients who are intravascularly volume depleted (eg, those treated with diuretics). Use a lower starting dose or decrease dose of diuretics
- Use with caution in treating patients whose renal function may depend on the activity of the renin-angiotensin-aldosterone system (severe CHF)
- Use with caution when initiating therapy in patients with heart failure. Hypotension may occur
- If pregnancy is expected, stop drug immediately. Maximal effect seen in 4-6 weeks
- Avoid therapy in children <1 year of age for hyotension, as it may impair renal development
- Risk of hypotension may occur in hypovolemic/hyponatremic patients, concomitant diuretics, dialysis, or during major surgery and anesthesia
- Therapy may cause hyperkalemia, hence monitor serum K+ periodically

- Hepatic impairment
- Renal artery stenosis
- Renal impairment
- Women of child bearing potential
- During and before anesthesia/surgery
- CHF
Supplemental Patient Information
- Adolescents who have attained menarche should be inquired regularly to confirm changes in menstrual pattern and the probability of pregnancy
Pregnancy Category:C, D (C in the first trimester and D in the second and third trimesters)
