Adult Dosing

Prevention of hypokalemia

Usual dose: PO 20 mEq/day

Treatment of potassium depletion

Usual dose: PO 40-100 mEq/day

Note:

Divide doses if >20 mEq/day is given in such a way that that no more than PO 20 mEq is given in a single dose
Adjust doses as per individuals need based on serum potassium levels
Do not take on empty stomach, consume with meals and with a glass of water or any other fluid
All dosages in the form of tablet except Klor-con M tablets are to be swallowed whole without crushing, chewing or sucking
For patients having difficulty in swallowing Klor-con M tablets break the tablet in half and take each half separately with a glass of water or prepare a water suspension
For patients having difficulty in swallowing capsules sprinkle the contents of the capsule onto a spoonful of soft food such as applesauce or pudding swallow immediately without chewing followed by a glass of cool water or juice
Immediately use any microcapsule/food mixture, do not store for future use

Pediatric Dosing

Safety and effectiveness in pediatric patients have not been established

Prevention of hypokalemia

Usual dose: 1-2 mEq/kg/day in qd-bid divided doses (Of label dosing)

Intestinal, gastric ulceration and bleeding with extended-release preparations have occurred, reserve for patients who cannot tolerate or refuse to take liquid or effervescent potassium preparations and there is a problem of compliance with these preparations
On administration of potassium salts hyperkalemia and cardiac arrest have occurred in patients with impaired mechanisms for excreting potassium
Fatal asymptomatic hyperkalemia have occurred
Carefully monitor serum potassium concentration and consider appropriate dosage adjustment in patients with chronic renal disease
Avoid concomitant administration of potassium salts and a potassium-sparing diuretic
Carefully monitor patients receiving ACE inhibitors during administration of potassium as concomitant use will produce some amount of potassium retention
Ulcerative or stenotic lesions of the gastrointestinal tract, increased frequency of small bowel lesions and gastric, duodenal lesions have occurred
Immediately discontinue on possibility of ulceration, obstruction, or perforation signified by severe vomiting, abdominal pain, distention, or gastrointestinal bleeding
Treat hypokalemia in patients with metabolic acidosis with an alkalinizing potassium salt
During diagnoses bear in mind that acute alkalosis can produce hypokalemia in the absence of a deficit in total body potassium while acute acidosis can increase serum potassium concentration in the normal range even in the presence of a reduced total body potassium
Artifactual elevations can occur after improper venipuncture technique or as a result of in-vitro hemolysis of the sample withdrawn for analysis of plasma potassium
Perform electrocardiogram and monitor serum electrolytes, acid-base balance and the clinical status of the patient during therapy
Monitor creatinine at baseline and periodically thereafter in geriatric patients

Use cautiously in:

Renal impairment
Cardiovascular disease
Diabetic acidosis
Diabetic gastroparesis
Acute dehydration
Extensive tissue breakdown as in severe burns
Adrenal insufficiency
Systemic acidosis
Extensive tissue breakdown
Concomitant use with potassium-sparing diuretic
Concomitant use with anticholinergic agents

Pregnancy Category:C

Breastfeeding: Probably safe; normal potassium content of human milk is about 13 mEq/liter, oral potassium forms part of potassium pool in body, as long as body potassium is within limit the contribution of potassium chloride supplementation will have less or no effect on the level in human milk and will not have an adverse reaction on fetus. Manufacturer advises caution
