Adult Dosing

Stimulation of pancreatic secretions, including bicarbonate, in the diagnosis of pancreatic exocrine dysfunction

• 0.2 mcg/kg IV over 1 min

Stimulation of gastrin secretion to aid in the diagnosis of gastrinoma

• 0.4 mcg/kg IV over 1 min

Endoscopic retrograde cholangiopancreatography (ERCP)

• 0.2 mcg/kg IV over 1 min

Note:

• Give a test dose of secretin 0.2 mcg (16 mcg/vial) and 0.4 mcg (40 mcg/vial) IV to test for possible allergy before administering secretin

Pediatric Dosing

• Safety and effectiveness in pediatric patients have not been established

• Stimulation of pancreatic secretions, including bicarbonate, in the diagnosis of pancreatic exocrine dysfunction
• Stimulation of gastrin secretion to aid in the diagnosis of gastrinoma
• During endoscopic retrograde cholangiopancreatography (ERCP) to facilitate the identification of the ampulla of Vater and accessory papilla [Chirhostim]

Hepatic Dose Adjustment

• Administer a test dose to identify allergic reaction to secretin, especially in patients with a history of atopic allergy and/or asthma. If no allergic reaction is noted after 1 min, administer the recommended dose
• Results of secretin stimulation tests should be interpreted with caution in patients who have undergone vagotomy or who have inflammatory bowel disease, as they respond less to secretin stimulation and may give false positive result of pancreatic diseases
• Patients with alcoholic or other liver disease may present a greater-than-normal volume response to secretin stimulation and may mask the coexisting pancreatic diseases. Thus the results of secretin stimulation tests should be interpreted with caution in these patients

• Concomitant anticholinergic agents

• Diagnostic Agent; binds to the secretin receptor found on the lining of S cells in the duodenum and G cells in the stomach leading to the secretion of bicarbonate from bicarbonate producing organs or reduction of the secretion of gastrin

• Excretion: After 0.4 mcg/kg IV administration, plasma concentration rapidly declines to baseline secretin levels within 90 to 120 min
