Pediatric Dosing
Respiratory Distress Syndrome
- 100 mg of phospholipids 4mL/kg birth weight/dose, by intratracheal administration; max 4 doses, at 6 hour intervals in the first 48 hours after birth
- Each dose is divided in to 4 quarter doses to ensure homogenous distribution throughout the lungs
- Each quarter-dose is administered with the infant in a different position
- Recommended positions are:
- Head and body inclined 5-10 degree down, head turned to the right
- Head and body inclined 5-10 degree down, head turned to the left
- Head and body inclined 5-10 degree up, head turned to the right
- Head and body inclined 5-10 degree up, head turned to the left
- Rescue: Give 1st dose as soon as possible after infant is placed on mechanical ventilation for treatment of RDS
- Prevention: Give 1st dose as soon as possible after birth, preferably within 15 minutes. Avoid suctioning for 1 hour after treatment, unless significant airway obstruction occurs
- Retreatment is determined by continued respiratory distress. Get radiographic confirmation of RDS in those infants who were given preventive dose before giving additional doses

- Intended for intratracheal use only
- Administer in a highly supervised clinical setting
- May rapidly affect oxygenation and lung compliance. Therefore, frequent and careful clinical observation and monitoring of systemic oxygenation are essential to avoid hyperoxia
- Transient episodes of bradycardia and decreased oxygen saturation may occur during dosing; interrupt dosing if bradycardia or oxygen desaturation occurs; treat; resume after stabilization
- Monitor oxygenation, carbon dioxide, and clinical condition frequently and carefully
- Increased probability of post-treatment nosocomial sepsis observed
- Use of the drug in infants less than 600 g birth weight or greater than 1750 g has not been established
- Rales and moist breath sounds can occur transiently after administration and do not indicate overdosage; remedial action is not required unless clear-cut signs of airway obstruction are present
- No data is available on the effects of doses other than 100 mg phospholipids/kg, more than four doses, dosing more frequently than every 6 hours, or administration after 48 hours of age
