Adult Dosing
Duodenal ulcers
- Recommended dose: 15 mg PO qd x 4 wks
- Maintenance dose: 15 mg PO qd
Benign gastric ulcer
- Recommended dose: 30 mg PO qd x upto 8 wks
NSAID-associated gastric ulcer
- Recommended dose: 30 mg PO qd x 8 wks
- To reduce risk of gastric ulcer: 15 mg PO qd x 12 wks
Gastroesophageal reflux disease (GERD)
- Symptomatic GERD
- Recommended dose: 15 mg PO qd x up to 8 wks
- Erosive esophagitis
- Recommended dose: 30 mg PO qd x up to 8 wks
- If recurrence or incomplete healing; repeat x 8 wks
Hypersecretory conditions including Zollinger-Ellison syndrome
- Recommended dose: 60 mg PO qd
- >120 mg/day in divided doses
- Max: 90 mg PO bid
Note Administer dose before meals. ODT should not be crushed, chewed and cut
Pediatric Dosing
Short-term treatment of symptomatic GERD and erosive esophagitis
- 1-11 yrs
- 30 kg
- Recommended dose: 15 mg PO qd x up to 12 wks
- Max: 30 mg bid
- >30 kg
- Recommended dose: 30 mg PO qd x up to 12 wks
- Max: 30 mg bid
- 12-17 yrs
- Nonerosive GERD
- Recommended dose: 15 mg PO qd x up to 8 wks
- Erosive esophagitis
- Recommended dose: 30 mg PO qd x up to 8 wks
- Max: 30 mg bid
Maintenance of healing of erosive esophagitis
- Recommended dose: 15 mg PO qd
Hypersecretory conditions including Zollinger-Ellison syndrome
- Recommended dose: 60 mg PO qd

- Dose can be increased after 2 or more weeks of treatment
- Administer dose before meals

- Symptomatic response with lansoprazole does not preclude the presence of gastric malignancy
- PPI therapy including lansoprazole may increase risk of Clostridium difficile associated diarrhea, particularly in hospitalized patients
- Increased risk for osteoporosis-related fractures of the hip, wrist or spine have been associated with high dose or long-term proton pump inhibitor (PPI) therapy
- Hypomagnesemia has been reported with prolonged usage
- Use of PPIs with high dose methotrexate may increase and prolong serum levels of methotrexate and its metabolite leading to methotrexate toxicities. Temporary withdrawal of the PPI must be considered in patients who are on high-dose of methotrexate
