Adult Dosing

Hypertension

• Start 600 mg/day PO when used as monotherapy in patients who are not volume depleted
• May be increased to 800 mg/day
• Max: 800 mg/day divided qd-bid
• Note: Divide dose bid if effect is inadequate at trough with qd dosing; max response occurs in 2-3 wks

Heart failure (Non-FDA Approved)

• Note that similar dosing to that used for hypertension is typically used for this indication

Pediatric Dosing

• Safety and effectiveness in pediatric population have not been established

• Hypersensitivity to any of the ingredient of the product
• Pregnancy (see warnings)
• Co-administration of aliskiren in patients with diabetes

• Drugs that act directly on the renin-angiotensin system may cause fetal/neonatal morbidity and mortality when used during pregnancy. When pregnancy is detected, the drug should be discontinued as soon as possible

Renal Dose Adjustment

• Moderate-severe impairment: Max 600 mg/24 hrs
• Hemodialysis: not defined

Hepatic Dose Adjustment

• Hepatic impairment: No dose adjustments

• Eprosartan can cause fetal/neonatal morbidity/mortality if used during pregnancy. Discontinue as soon as possible once pregnancy is detected. Drugs that act on the renin-angiotensin system used during the second and third trimesters have been associated with fetal and neonatal injury and death. If oligohydramnios is observed, discontinue unless it is considered lifesaving to the mother
• Correct volume or salt depletion before starting therapy with eprosartan. Symptomatic hypotension may occur in patients with an inactivated renin-angiotensin system
• In patients whose renal function may depend on the activity of the renin-angiotensin-aldosterone system (eg, severe CHF), use may be associated with oliguria, progressive azotemia, acute renal failure, and death
• NSAID use should be limited or avoided, with continuous BP monitoring
• In CHF, NSAIDs may be associated with increased fluid retention and edema

Cautions: Use cautiously in

• Renal impairment
• Renal artery stenosis
• Hepatic impairment
• Volume- or salt-depleted patients or patients receiving large doses of diuretics
• Black patients (efficacy decreased)
• Women of childbearing potential
• Patients with primary renal disease or heart failure

Supplemental Patient Information

• May cause dizziness, fainting, or light-headedness (use with caution in tasks requiring alertness until response to drug is known)
• Postural hypotension (use with caution when rising from sitting or lying position or climbing stairs)

• M & N: Hypertriglyceridemia, hyperkalemia, hypokalemia
• HEMA: Leukopenia, neutropenia
• MISC: Angioedema, dependent edema, rhabdomylosis, viral infection

• Mechanism of action: Angiotensin II, formed from angiotensin I by the action of angiotensin-converting enzyme (ACE, kininase II), causes vasoconstriction and synthesis and release of aldosterone. Eprosartan selectively blocks the binding of angiotensin II to the AT1 receptor. The affinity towards AT1 receptor is 1,000 times greater than to the AT2 receptor

• Excreted in feces (90%) and urine (7%)
