Orthostatic hypotension
Hepatorenal Syndrome [Non-FDA approved]
Note:
- Dose should be taken during the daytime hours when the patient needs to be upright
- There should be at least 4-hr interval between doses; shortly before or upon arising in the morning, midday and late afternoon (not later than 6 pm)
- If required, doses may be given at 3-hr intervals to control symptoms, but not more frequently
- Should not be given after the evening meal or <4 hrs before bedtime
- Monitor supine and standing blood pressure regularly during therapy

- Marked elevation of supine arterial blood pressure can occur (BP >200 mmHg) with midodrine therapy; elevations, such as these, were seen in patients with elevated pre-treatment systolic blood pressures (mean 170 mmHg)
- There is no experience in treating patients with initial supine systolic pressure >180 mmHg; midodrine use in such patients is not recommended
- Monitor supine and sitting blood pressures in patients maintained on midodrine
- Promptly discontinue therapy if supine hypertension occurs or persists; carefully monitor blood pressure during co-administration with vasoconstrictive agents such as phenylephrine, ephedrine, dihydroergotamine, pseudoephedrine or phenylpropanolamine
- Bradycardia may occur during therapy due to vagal reflex. Patients who develop signs or symptoms signifying bradycardia should be advised to discontinue therapy and should be re-evaluated
- Monitor renal and hepatic functions prior to initiation of therapy and subsequently, as required

- Hepatic impairment
- Hypertension
- Concomitant use with cardiac glycosides
- Use with psychopharmacologic agents
- Co-administration with beta-blockers
- Agents that reduce heart rate
- Urinary retention problems
- Diabetes mellitus
- History of visual problems
- Patients on fludrocortisone
Supplemental Patient Information
- Inform patients that certain over-the-counter products such as cold remedies and diet aids can elevate blood pressure and should be used carefully during therapy, as they may potentiate the pressor effects of midodrine
