Adult Dosing
Superficial bacterial infections of the external auditory canal; treatment of infections of mastoidectomy and fenestration cavities
- Using the calibrated dropper, instill 5 gtt into the affected ear tid-qid x 10 days; repeat, if necessary, for the opposite ear
- Alt: Insert a cotton wick into the canal and then saturate the cotton with the suspension; moisten the wick by adding further solution q4 hrs. Replace the wick q24 hrs
Note: - Before initiating therapy, the external auditory canal should be thoroughly cleansed and dried with a sterile cotton applicator
Pediatric Dosing

- Neomycin may cause permanent sensorineural hearing loss due to cochlear damage. Prolonged use may increase the risk of ototoxicity; therefore, duration of therapy should be limited to 10 consecutive days
- Closely monitor patients being treated with eardrops containing neomycin. Use cautiously in patients with a perforated tympanic membrane
- Cutaneous sensitization, manifested by low-grade reddening with swelling, dry scaling and itching, may occur with the use of neomycin-containing products. Promptly evaluate for such signs; advise patients to discontinue the product if sensitivity or irritation occurs and to avoid the use of neomycin-containing products thereafter
- Prolonged use of therapy may result in overgrowth of nonsusceptible organisms, including fungi. If the infection is not improved after 1 week, cultures and susceptibility tests should be repeated to verify the identity of the organism and to determine whether therapy should be changed
- Do not continue this treatment for longer than 10 days
- Allergic cross-sensitivity may occur, which could prevent future use of any or all of the aminoglycoside antibiotics (kanamycin, paromomycin, streptomycin, and possibly gentamicin)
- Bacterial resistance to components of the product may develop

Supplemental Patient Information
- Advise patients to avoid contaminating the dropper with material from the ear, fingers, or other source
- Advise patients to discontinue treatment immediately and contact their physician, if sensitization or irritation occurs
- Instruct patients not to use in the eyes
- Advise patients not to heat the suspension above body temperature in order to avoid loss of potency
Pregnancy Category:C
Breastfeeding: Safety unknown. Topical hydrocortisone has not been studied during breastfeeding. Short-term application of topical corticosteroids is not expected to pose a risk to the breastfed infant by passage into breastmilk. Very low levels of neomycin are excreted in breastmilk following otic administration and present negligible risk to the nursing infants. This information is based upon LactMed database (available at http://toxnet.nlm.nih.gov/cgi-bin/sis/htmlgen?LACT last accessed 2 June 2011). As per manufacturer's data, hydrocortisone and colistin sulfate are excreted in human milk following oral administration. As systemic absorption of these drugs may occur when used topically, manufacturer advises caution when used by a nursing woman.
