Adult Dosing
Hypertension
Tenex
- Initial dose 1 mg PO qd at bedtime
- May be increased if necessary at 3-4 wk intervals up to 2-3 mg qd at bedtime
- Max: 3 mg/day
Pediatric Dosing
Attention Deficit Hyperactivity Disorder (ADHD)
Intuniv
- Safety and effectiveness in pediatric patients <6 yrs of age have not been established
- Child 617yrs (monotherapy and adjunctive therapy with a psychostimulant): Initially 1 mg qd; titrate by 1 mg/day at 1-wk intervals
- Max: 4 mg qd
Notes: - Re-evaluate periodically. When discontinuing, withdraw gradually by 1 mg q37 days
- If switching from immediate-release tablet therapy is required, discontinue immediate-release tablet and titrate with extended-release tablet according to the recommended dose schedule
- Do not give with high-fat meals
- Tablets should not be crushed, chewed or broken before swallowing as this will increase the rate of guanfacine release

- It is not indicated in the treatment of acute hypertension with toxemia of pregnancy
- Avoid abrupt cessation due to risk of rebound hypertension. Taper dose gradually to discontinue
- Decreases in BP and heart rate were usually modest and asymptomatic; however, hypotension and bradycardia may occur
- Monitor heart rate and BP before initiating the therapy, following dose increases, and periodically while on therapy
- Do not use guanfacine in patients concomitantly taking other guanfacine-containing products (e.g., Tenex)
- Hallucinations reported in children with ADHD receiving guanfacine
Cautions: Use cautiuosly in
- Renal impairment
- Hepatic Impairment
- Severe coronary insufficiency
- Recent myocardial infarction
- Cerebrovascular disease
- Geriatric population (use lower initial doses in view of decreased hepatic, cardiac or renal function)
- History of hypotension, heart block, bradycardia/cardiovascular disease (may decrease BP and heart rate)
- History of syncope
- Dehydration
- Patients treated concomitantly with antihypertensives
Supplemental Patient Information
- Advise patients to avoid becoming dehydrated or overheated
- Advise patients against operating heavy equipment or driving until they know how they respond to treatment with guanfacine
- Advise patients to avoid use with alcohol
