Adult Dosing

Active pulmonary and extrapulmonary tuberculosis

• Start 250 mg PO bid for 2 wks, then as tolerated 250 mg PO Q6-8H. [MAX: 1000 mg/day]

Acute urinary tract infections

• 250 mg PO BID X 14 days [Max: 1000 mg/day]

Pediatric Dosing

• Safety and effectiveness in pediatric patients have not been established, some references have advised 10 to 20 mg/kg/day in two divided doses

Renal Dose Adjustment

• Mild to moderate impairment: Reduce dose; dose adjustment not defined
• Severe impairment: Contraindicated if creatinine clearance less than 50 ml/min

Hepatic Dose Adjustment

• Reduce dosage or suspend therapy if the patient develops allergic dermatitis or symptoms of CNS toxicity
• Use with caution in alcoholics as the risk of convulsions is increased in chronic alcoholics
• Monitor patients by hematologic, renal excretion, blood level, chest X-ray, and liver function studies
• Before starting therapy, obtain cultures to determine the organism's susceptibility to the drug
• Therapy with cycloserine has been associated with vitamin B12 and/or folic acid deficiency, megaloblastic anemia, and sideroblastic anemia. Institute appropriate therapy if anemia develops
• Monitor CrCl/BUN (urea), CBC, LFTs periodically during therapy; monitor serum drug levels in patients receiving > 500 mg/day qwk if renal impairment or if signs of toxicity are present
