Adult Dosing
Hypoparathyroidism adjunct
- 50,000 - 200,000 IU PO daily (use with calcium supplements)
Vitamin D resistant rickets
- 12,000 - 500,000 IU PO daily
Familial hypophosphatemia
- 10,000 - 60,000 IU PO daily (use with phosphorus 12 g/day)
Notes:
- 1 mg of ergocalciferol is equivalent to 40,000 USP units
- Dosage must be individualized to maintain serum calcium concentrations of 910 mg/dL

- In infants with idiopathic hypercalcemia, hypersensitivity to vitamin D may be one etiologic factor, vitamin D is strictly contraindicated in these patients
- Evaluate vitamin D administration from fortified foods, dietary supplements, self-administered & prescription drug sources; adjust dose accordingly
- Readjust the therapeutic dose as soon as there is clinical improvement, dosage level should be individualized and great care exercised to prevent serious toxic effects, as in vitamin D resistant rickets the range between therapeutic and toxic dose is very narrow
- Monitor frequently the blood calcium levels when high therapeutic doses are used
- In the treatment of hypoparathyroidism, intravenous calcium, parathyroid hormone, and/or dihydrotachysterol may be required
- Maintain normal serum phosphorus level by dietary phosphate restriction and/or administration of aluminum gels as intestinal phosphate binders in patients with hyperphosphatemia, as seen in renal osteodystrophy, to prevent metastatic calcification
- Ensure adequate dietary calcium for clinical response to vitamin D therapy

- Renal impairment
- Hyperphosphatemia
- Renal calculus
- Concurrent thiazide diuretics (hypothyroid patients)

Breastfeeding: Use with caution, monitor infant serum calcium levels.
