Adult Dosing

Infections, bacterial

• 250-500 mg PO tid
• Alt: 375 -500 Extended Release TABS PO bid
• Max: 2 g/day
• Use for 7-10 days

• ER form to be taken with food
• Do not cut/crush/chew ER form

Pediatric Dosing

Infections, bacterial

>1 month

• Immediate release
• 20-40 mg/kg/day PO divided Q8-12H
• Max: 1 g/day

• Treatment of the following bacterial infections caused by susceptible microorganisms
• Respiratory tract infections
• Otitis media
• Skin and skin structure infections
• Urinary tract infections

• Hypersensitivity to cephalosporins

• Serious hypersensitivity reactions have been reported with the use of cefaclor. Epinephrine and other emergency measures must be available for management of acute reactions. Administer drug with caution to penicillin-sensitive patients because of possible cross-reactivity
• Clostridium difficile associated diarrhea which may vary in severity from mild to fatal colitis, has been reported with use of cefaclor
• Obtain culture and sensitivity tests before prescribing cefaclor. Prescribing cefaclor in the absence of a proven or strongly suspected bacterial infection increases the risk of the development of drug-resistant bacteria
• Prolonged use may result in the overgrowth of nonsusceptible organisms

Cautions: Use cautiously in

• Renal impairment
• Hx of GI disease
• Hx of antibiotic associated colitis
• Concurrent nephrotoxic agents
• Seizure disorder

Pregnancy Category:B

Breastfeeding: Limited information indicates that maternal cefaclor produce low levels in milk that are not expected to cause adverse effects in breastfed infants. This drug is compatible and considered safe with breastfeeding based upon data from LactMed database (available at http://toxnet.nlm.nih.gov/cgi-bin/sis/htmlgen?LACT last accessed 12 May 2010). Manufacturer advises caution.

• CNS: Seizures, agitation, nervousness, insomnia, confusion, hypertonia,dizziness, hallucinations, and somnolence
• GI: pseudomembranous colitis, diarrhea , nausea, vomiting, elevated LFTs, transient hepatitis, cholestatic jaundice
• Derm: Rashes, urticaria, pruritus, exfoliative dermatitis
• GU: Nephrotoxicity, vaginitis
• HEMA: Agranulocytosis, eosinophilia, neutropenia, thrombocytopenia, hemolytic anemia
• MISC: Allergic reactions including Stevens-Johnson syndrome, erythema multiforme and toxic epidermal necrolysis; serum sickness–like reactions, anaphylaxis, superinfection

• Bactericidal; binds to bacterial cell wall membrane; inhibits cell wall mucopeptide synthesis
• Not metabolized; CYP450: unknown
• Primarily excreted: (60-80% unchanged)
• Half-life: 1 hr (2.3-2.8 hrs in ESRD)
