Adult Dosing
Type II diabetes mellitus (NIDDM)
- Starting and maintenance dose: 120 mg PO tid before meals
- May give 60 mg PO tid in patients who are close to goal HbA1C when treatment is initiated
Notes:- Dose of 60-120 mg PO tid can be used alone or in combination with metformin or thiazolidinedione
- Should be taken 1-30 minutes before meals; skip dose if meal is skipped to reduce the risk of hypoglycemia

- Nateglinide may produce hypoglycemia; geriatric patients, malnourished patients, and those with adrenal or pituitary insufficiency or severe renal impairment are at greater risk of hypoglycemia
- Increased risk of hypoglycemia may be due to strenuous physical exercise, ingestion of alcohol, insufficient caloric intake on an acute or chronic basis, or combinations with other oral antidiabetic agents
- Hypoglycemia may be difficult to recognize in patients having autonomic neuropathy or those taking beta-blockers
- Nateglinide should be taken before meals to reduce the risk of hypoglycemia
- Transient loss of glycemic control may result with fever, trauma, infection, or surgery; discontinue nateglinide and initiate insulin therapy
- Assess therapy response periodically using glucose values and glycosylated hemoglobin levels
Cautions: Use cautiously in
- Moderate-severe hepatic impairment
- Malnutrition
- Adrenal insufficiency
- Autonomic neuropathy
- Elderly patients

- Educate patients about the risks and management of hypoglycemia
Pregnancy Category:C
Breastfeeding: Because no information is available on the use of nateglinide during breastfeeding, an alternate drug may be preferred, especially while nursing a newborn or preterm infant. Monitoring of the breastfed infant's blood glucose is advisable during maternal therapy with hypoglycemic agents. It is unknown whether nateglinide is excreted in human milk. As per manufacturer's data, nateglinide should not be administered to a nursing woman.
