Adult Dosing
Major depressive disorder
- Start 50 mg PO qd
- Increase qwk up to 200 PO qd, depending upon response
- Range 50-200 mg PO qd
- Max: 200 mg PO qd
Obsessive compulsive disorder
- Start 50 mg PO qd
- Increase qwk up to 200 mg PO qd, depending upon response
- Range 50-200 mg PO qd
- Max: 200 mg PO qd
Post traumatic stress disorder
- 25 mg PO qd x 7 days, then increase to 50 mg PO qd
- May then be further increased if needed at intervals of at least 7 days
- Range 50-200 mg PO qd
- Max: 200 mg PO qd
Panic disorder
- 25 mg PO qd x 7 days, then increase to 50 mg PO qd
- May then be further increased if needed at intervals of at least 7 days
- Range 50-200 mg PO qd

Social anxiety disorder
- 25 mg PO qd x 7 days, then increase to 50 mg PO qd
- May then be further increased if needed at intervals of at least 7 days

Premenstrual dysphoric disorder
- Daily dosing
- 50 mg PO qd, may increase 50 mg/day each cycle
- Max: 150 mg PO qd
- Luteal phase dosing
- 50 mg PO qd on cycle days 15-28
- Max: 100 mg PO qd
- Note: If a 100 mg PO qd dose has been established, a 50 mg/day titration step for three days should be utilized at the beginning of each luteal phase dosing period
Notes:- Taper dose to discontinue
- Sertraline must be adminstered once daily, either in the morning or evening
Pediatric Dosing
Obsessive-compulsive disorder
- 6-12 yrs
- Start 25 mg PO qd; may incr. 25-50 mg/day qwk up to 200 PO qd

- 13-17 yrs
- Start 50 mg PO qd; may incr. 25-50 mg/day qwk up to 200 PO qd

- Before initiating treatment with an antidepressant, screen patients to determine if they are at risk for bipolar disorder
- Cases of serious sometimes fatal reactions have been reported in patients receiving sertraline in combination with a monoamine oxidase inhibitor (MAOI)
- Neuroleptic Malignant Syndrome (NMS) has been reported in patients taking antipsychotic drugs. Signs and symptoms include hyperpyrexia, muscle rigidity, altered mental status, irregular pulse or BP, tachycardia, diaphoresis, and cardiac arrhythmias
- Activation of mania/hypomania occurs infrequently in patients taking SSRIs
- Bleeding episodes have been reported in patients taking psychotropic drugs that interfere with serotonin reuptake. Concomitant use of aspirin, nonsteroidal anti-inflammatory drugs and warfarin increases risk of bleeding
- Sertraline may cause a decrease in serum uric acid
- Significant weight loss has also been reported
- Monitor and observe closely for clinical worsening, suicidality, and unusual changes in behavior, especially during the initial few months, or at times of dose changes
- Avoid abrupt withdrawal of the drug
Cautions: Use cautiously in
- Pregnancy >20 wks gestation
- History of seizure disorder
- Intake during pregnancy can lead to neonatal persistent pulm. HTN (>20 wk gestation), neonatal serotonin syndrome (3rd trimester), and neonatal withdrawal (3rd trimester)
Supplemental Patient Information
- Advise caregivers to monitor patients for the emergence of agitation, irritability, unusual changes in behavior, as well as the emergence of suicidality, and to report such symptoms immediately to health care providers

Breastfeeding: Sertraline is excreted in breastmilk in low levels and is usually not detected in the serum of the infant. The weakly active metabolite desmethyl sertraline is detectable in low levels in infant serum.
