Adult Dosing
Moderate to severe Alzheimer's dementia
- Initial dose: 5 mg PO qd
- Increase the dose in increments of 5 mg qwk to a maximum of 20 mg/day divided bid
- Max: 20 mg/day (10 mg bid)
Moderate to severe Alzheimer's disease (Namenda XR)
- Initial dose: 7 mg PO qd
- Increase the dose in increments of 7 mg/wk to a maximum of 28 mg qd
- Max: 28 mg PO qd
Note:
- Patients receiving Namenda tabs 10 mg PO bid can switch to Namenda XR Caps 28 mg PO qd; following the last dose of 10 mg Namenda tabs
- Patients receiving Namenda tabs 5 mg PO bid can switch to Namenda XR Caps 14 mg PO qd; following the last dose of 5 mg Namenda tabs
Pediatric Dosing
- Safety and effectiveness in pediatric patients have not been established

Renal Dose Adjustment (Based on CrCl)
- Mild-moderate renal impairment: No dose adjustments
- 5-29 mL/min: 5 mg PO bid (Namenda)
- 5-29 mL/min: 14 mg/day (Namenda XR)
Hepatic Dose Adjustment
- Mild-moderate hepatic impairment: No dose adjustments
- Severe impairment: Caution advised
- Conditions that raise urine pH may decrease the urinary elimination of memantine leading to elevated plasma levels of memantine
- Rare events of seizures have been reported with memantine
Cautions: Use cautiously in
- Severe hepatic impairment
- Severe renal impairment
- Renal tubular acidosis
- Hx of urinary tract infection
- History of seizure disorder
- Pregnancy
- Lactation
