Adult Dosing
Pain and inflammation post cataract surgery
- Instill 1 gtt to the affected eye(s) tid, starting 1 day prior to cataract surgery, continued on the day of surgery, and then x 2 wks post-op period
Notes:- Shake well before use
- Nepafenac can be administered in conjunction with other topical ophthalmic medications, but at least 5 minutes apart
Pediatric Dosing

- NSAIDs, including nepafenac, have the potential for increasing bleeding time due to interference with thrombocyte aggregation; ocular application of NSAIDs may cause increased bleeding of ocular tissues in conjunction with ocular surgery. Hence, nepafenac should be used with caution in patients with known bleeding tendencies or who are receiving other medications that may prolong bleeding time
- Topical NSAIDs such as nepafenac may slow or delay healing; such delayed healing also occurs while using topical corticosteroids. Hence, concomitant use of topical NSAIDs and topical steroids may increase the potential for healing problems
- Application of topical NSAIDs may result in keratitis. In some susceptible patients, prolonged use may result in epithelial breakdown, corneal thinning, corneal erosion, corneal ulceration or corneal perforation, which may be sight threatening. It is advised to discontinue therapy in patients with evidence of corneal epithelial breakdown associated with the use of topical NSAIDs and consider close monitoring for corneal health
- Post-marketing reports have suggested that nepafenac use more than 1 day prior to surgery or beyond 2 wks post surgery may increase patient risk and severity of corneal adverse events
- Do not administer to patients using contact lenses

- Dry eye syndrome
- Rheumatoid arthritis

Supplemental Patient Information
- Inform patients using contact lenses not to use the drug while wearing contact lens
- Instruct patients not to touch the container tip to any surface or to eye structures in order to avoid possible contamination of the solution; serious damage to the eye and subsequent vision loss may result from using contaminated solutions
- Advise patients to promptly report to their physicians if they develop intercurrent ocular condition or undergo any ocular surgery
Pregnancy Category:C
Breastfeeding: Safety unknown. As per manufacturer's data, it is not known whether nepafenac is excreted in human milk. Because many drugs are excreted in human milk, caution should be exercised while administering this drug to a nursing woman.
