Adult Dosing
Prevention of Nausea and Vomiting
Associated with highly emetogenic cancer chemotherapy
- 24 mg PO 30 min before chemotherapy as a single dose
Associated with moderately emetogenic chemotherapy
- Initial dose : 8 mg TABS/ODT/Soluble Film or 10 mL solution PO 30 min before chemotherapy
- Repeat 8 mg PO after 8 hrs of the first dose
- Then 8 mg PO bid x 1-2 days after completion of chemotherapy
Prevention of radiation-induced nausea and vomiting
Total body irradiation
- 8 mg TABS/ODT/Soluble Film or solution PO 1-2 hrs before each fraction of radiation
Single high dose fraction abdominal radiation
- 8 mg TABS/ODT/Soluble Film or solution PO 1-2 hrs before radiation
- Followed by 8 mg or 10 mL solution tid x 1-2 days after completion of radiotherapy
Daily fractionated abdominal radiation
- 8 mg TABS/ODT/Soluble Film or solution PO 1-2 hrs before radiation
- Subsequent doses: 8 mg PO q8 hrs after first dose for each day radiotherapy
Prevention of postoperative nausea and vomiting
- 16 mg TABS/ODT/Soluble Film or solution PO 1 hr prior to anesthesia

Pediatric Dosing
Prevention of Nausea and Vomiting associated with moderately emetogenic chemotherapy
- Child: 4-11 yrs
- 4 mg TABS/ODT/Soluble Film or solution PO 30 min before chemotherapy
- Repeat 4 mg PO after 4 hrs and 8 hrs of first dose
- Then 4 mg PO tid x 1-2 days after completion of chemotherapy
- Children 12 yrs
- Initial dose : 8 mg TABS/ODT/Soluble Film or solution PO 30 min before chemotherapy
- Repeat 8 mg PO after 8 hrs of the first dose
- Then 8 mg PO bid x 1-2 days after completion of chemotherapy
Nausea and vomiting [Non-FDA Approved]
- 0.1-0.15 mg/kg PO q4-8 hours PRN; max 4 mg
[Outline]
- Ondansetron causes hypersensitivity reaction in patients who have exhibited hypersensitivity to other selective 5-HT3 receptor antagonists
- Ondansetron does not stimulate gastric or intestinal peristalsis; it should not be used instead of nasogastric suction
- It may masks a progressive ileus and/or gastric distention when used following abdominal surgery or chemotherapy induced nausea and vomiting
- Concomitant use of apomorphine is contraindicated, as it causes profound hypotension and loss of consciousness
- QT prolongation have been reported, primarily with the injection
- Oral disintegrating tablets contain phenylalanine
