Adult Dosing
Beriberi
- Start 10-20 mg IM tid x 2 wks; follow with 5-30 mg PO daily x1 month
Wet beriberi with CHF
Wernicke's encephalopathy
- 100 mg IV x1; followed by 50-100 mg IM/IV daily
Pediatric Dosing
- Safety and effectiveness in pediatric patients have not been established

- Repeated parenteral administration may lead to possible severe hypersensitivity reactions/anaphylaxis
- Prior to parenteral administration of therapeutic doses of thiamine administer an intradermal test dose in patients who may be sensitive to the drug
- Administer cautiously in premature neonates or if kidney function is impaired because some preparations contain aluminum, which may be toxic and aluminum may reach toxic levels with prolonged parenteral administration
- Monitor for CNS and bone toxicity or tissue loading during the therapy in patients receiving parenteral levels of aluminum at greater than 4 to 5 mcg/kg/day
- Maintain a patent airway and the use of epinephrine, oxygen, vasopressors, steroids and antihistamines if anaphylactic reactions occur
