Adult Dosing

Healing of erosive esophagitis (EE)

• 60 mg PO qd up to 8 wks

Maintenance of healed EE

• 30 mg PO qd x up to 6 months

Symptomatic non-erosive GERD

• 30 mg PO qd x 4 wks

• Safety and effectiveness in pediatric patients have not been established

Renal Dose Adjustment

• Renal impairment: No dose adjustments

Hepatic Dose Adjustment

• Mild hepatic impairment (Child-Pugh class A): No dose adjustments
• Moderate hepatic impairment (Child-Pugh class B): 30 mg/day
• Child-Pugh class C: Dose adjustments not defined

• Symptomatic response with dexlansoprazole does not preclude the presence of gastric malignancy
• Therapy may be associated with an increased risk for osteoporosis-related fractures of the hip, wrist or spine; this risk increases with high-dose or long-term proton pump inhibitor (PPI) therapy
• Use of PPIs with high dose methotrexate may increase and prolong serum levels of methotrexate and its metabolite leading to methotrexate toxicities. Temporary withdrawal of the PPI must be considered in patients who are on high-dose of methotrexate
• PPI therapy including dexlansoprazole may increase risk of Clostridium difficile associated diarrhea, particularly in hospitalized patients
